    Beyond the fundamental noise limit in coherent optical fiber links

    It is well known that temperature variations and acoustic noise affect ultrastable frequency dissemination along optical fiber. Active stabilization techniques are in general adopted to compensate for the fiber-induced phase noise. However, despite this compensation, the ultimate link performances remain limited by the so called delay-unsuppressed fiber noise that is related to the propagation delay of the light in the fiber. In this paper, we demonstrate a data post-processing approach which enables us to overcome this limit. We implement a subtraction algorithm between the optical signal delivered at the remote link end and the round-trip signal. In this way, a 6 dB improvement beyond the fundamental limit imposed by delay-unsuppressed noise is obtained. This result enhances the resolution of possible comparisons between remote optical clocks by a factor of 2. We confirm the theoretical prediction with experimental data obtained on a 47 km metropolitan fiber link, and propose how to extend this method for frequency dissemination purposes as well

    Correlated electron-hole plasma in organometal perovskites

    Organic-inorganic perovskites are a class of solution-processed semiconductors holding promise for the realization of low-cost efficient solar cells and on-chip lasers. Despite the recent attention they have attracted, fundamental aspects of the photophysics underlying device operation still remain elusive. Here we use photoluminescence and transmission spectroscopy to show that photoexcitations give rise to a conducting plasma of unbound but Coulomb-correlated electron-hole pairs at all excitations of interest for light-energy conversion and stimulated optical amplification. The conductive nature of the photoexcited plasma has crucial consequences for perovskite-based devices: in solar cells, it ensures efficient charge separation and ambipolar transport while, concerning lasing, it provides a low threshold for light amplification and justifies a favourable outlook for the demonstration of an electrically driven laser. We find a significant trap density, whose cross-section for carrier capture is however low, yielding a minor impact on device performance

    Among the symptoms of SARS-CoV-2, follicular conjunctivitis has become relevant. The conjunctiva acts as an open lymph node, reacting to the viral antigen that binds the epithelial cells, forming follicles of B cells with activated T cells and NK cells on its surface, which, in turn, talk to monocyte-derived inflammatory infected macrophages. Here, the NLRP3 inflammasome is a major driver in releasing pro-inflammatory factors such as IL-6 and caspase-1, leading to follicular conjunctivitis and bulbar congestion, even as isolated signs in the ‘asymptomatic’ patient

    Metformin-associated lactic acidosis and temporary ileostomy: a case report

    Lactic acidosis is a well-known complication of the anti-hyperglycemic biguanide agent metformin, especially in peculiar but not rare clinical conditions. Attempts to reduce the incidence of this adverse reaction have been enforced by national agencies over the years. The Italian Medicines Agency recently released a safety recommendation on prescribing the drug and with regard to the existence of several conditions that contraindicate drug continuation, such as dehydration, infection, hypotension, surgery or hyperosmolar contrast agent infusion, but the recommendation does not mention the increased risk related to stoma. The present case report is, to our knowledge, the first in the literature of metformin-associated lactic acidosis in a patient with a recently created ileostomy and low anterior resection for rectal cancer. CASE PRESENTATION: A 70-year-old Caucasian man who had undergone low anterior resection with total mesorectal excision and temporary loop ileostomy creation at our institution returned to our department 30 days later because of nausea, vomiting, diffuse abdominal pain and anuria of about 24 hours' duration. During his physical examination, the patient appeared dehydrated and had tachypnea and a reduced level of consciousness. His laboratory tests showed that he had acute kidney injury and severe lactic acidosis. CONCLUSION: An ileostomy puts patients at high risk for output losses that can lead to dehydration and electrolyte abnormalities. The assessment of the losses through the stoma, especially the ileostomy, should be added to the recommendations issued by pharmacovigilance societies. The present clinical case illustrates the need for clinicians on surgical wards to carefully evaluate patients before resuming metformin therapy and to provide appropriate information at discharge to patients with type 2 diabetes mellitus who have undergone ileostomy. Furthermore, this case report highlights the increasing need for more training of general physicians regarding both surgical and internal medicine problems that may arise in the post-operative course after major surgery in patients with co-morbidities

    Relationship between milk urea, blood plasma urea and body condition score in primiparous browsing goats with different milk yield level

    Abstract. The aim of this study was to investigate the relationships among milk urea, blood plasma urea, milk yield and body condition score (BCS) in primiparous goats fed at pasture. Ninety goats of Sarda breed were used and, on the basis of their yield level, divided in three groups of 30 animals each, low (LY), intermediate (IY) and high milk yield (HY). Daily milk yield, milk protein content, milk urea, plasma total protein and albumin, plasma urea and BCS were measured at monthly intervals from 45 days in milking (45 DIM) to 165 DIM. Milk yield level affected protein concentration of milk and plasma, whereas albumin showed no variation. Plasma and milk urea showed a high correlation (P<0.001) despite of the yield level; plasma urea was always lower than milk urea. BCS decreased on 75 DIM and again after 135 DIM, and it was not affected by the milk yield level. Because milk urea and plasma urea were closely correlated and not influenced by the yield level, the study pointed out that measurement of milk urea could be utilized to evaluate urea metabolism also for browsing goats

    A Large Area Fiber Optic Gyroscope on multiplexed fiber network

    We describe a fiber optical gyroscope based on the Sagnac effect realized on a multiplexed telecom fiber network. Our loop encloses an area of 20 km2 and coexists with Internet data traffic. This Sagnac interferometer achieves a sensitivity of about (10-8 rad/s)/sqrt(Hz), thus approaching ring laser gyroscopes without using narrow linewidth laser nor sophisticated optics. The proposed gyroscope is sensitive enough for seismic applications, opening new possibilities for this kind of optical fiber sensors

    Epidemiology of intensive care unit-acquired sepsis in Italy: results of the SPIN-UTI network

    BACKGROUND: Sepsis is the major cause of mortality from any infectious disease worldwide. Sepsis may be the result of a healthcare associated infection (HAI): the most frequent adverse events during care delivery especially in Intensive Care Units (ICUs). The main aim of the present study was to describe the epidemiology of ICU-acquired sepsis and related outcomes among patients enrolled in the framework of the Italian Nosocomial Infections Surveillance in ICUs - SPIN-UTI project. STUDY DESIGN: Prospective multicenter study. METHODS: The SPIN-UTI network adopted the European protocols for patient-based HAI surveillance. RESULTS: During the five editions of the SPIN-UTI project, from 2008 to 2017, 47.0% of HAIs has led to sepsis in 832 patients. Overall, 57.0% episodes were classified as sepsis, 20.5% as severe sepsis and 22.5% as septic shock. The most common isolated microorganisms from sepsis episodes were Acinetobacter baumannii, Klebsiella pneumoniae and Pseudomonas aeruginosa. The case fatality rate increased with the severity of sepsis and the mean length of ICU-stay was significantly higher in patients with ICU-acquired sepsis than in patients without. CONCLUSION: Our study provides evidence that ICU-acquired sepsis occurs frequently in Italian ICU patients and is associated with a high case fatality rate and increased length of stay. However, in order to explain these findings further analyses are needed in this population of ICU patient

    Development of an Optimization-Based Atomistic-to-Continuum Coupling Method

    Atomistic-to-Continuum (AtC) coupling methods are a novel means of computing the properties of a discrete crystal structure, such as those containing defects, that combine the accuracy of an atomistic (fully discrete) model with the efficiency of a continuum model. In this note we extend the optimization-based AtC, formulated in arXiv:1304.4976 for linear, one-dimensional problems to multi-dimensional settings and arbitrary interatomic potentials. We conjecture optimal error estimates for the multidimensional AtC, outline an implementation procedure, and provide numerical results to corroborate the conjecture for a 1D Lennard-Jones system with next-nearest neighbor interactions.Comment: 12 pages, 3 figure

    Stress-free states of continuum dislocation fields: Rotations, grain boundaries, and the Nye dislocation density tensor

    We derive general relations between grain boundaries, rotational deformations, and stress-free states for the mesoscale continuum Nye dislocation density tensor. Dislocations generally are associated with long-range stress fields. We provide the general form for dislocation density fields whose stress fields vanish. We explain that a grain boundary (a dislocation wall satisfying Frank's formula) has vanishing stress in the continuum limit. We show that the general stress-free state can be written explicitly as a (perhaps continuous) superposition of flat Frank walls. We show that the stress-free states are also naturally interpreted as configurations generated by a general spatially-dependent rotational deformation. Finally, we propose a least-squares definition for the spatially-dependent rotation field of a general (stressful) dislocation density field.Comment: 9 pages, 3 figure
